Case Managers - Concurrent Review & Prior Auth - 40 hrs/wk - Boston Metro Area

FT Telephonic Case Manager – Westwood, MA

 

Responsibilities: To provide telephonic case management services to short-term disability recipients on-site at a client location. Assessment of the individual’s personal and medical history, current status, prognosis, and current treatment plans. Serves as a liaison with the claims administration staff and treating provider staff to facilitate maximum benefit and promote recovery. Recognizes and addresses medical and non-medical issues which may adversely impact employee recovery. Qualifications: Registered Nurse, Current MA Licensure, CCM preferred, three or more year’s experience in disability injury management, from an Occupational Health, Utilization Review or Rehabilitation preferred. CASE MANAGER-Mount Auburn Cambridge I.P.A. Cambridge, MA

 

The Mount Auburn Cambridge Independent Practice Association is looking for an experienced Case Manager to assist our physicians in managing the care of our BCBS patients.  In this newly created position, the Case Manager will follow patients in the hospital, extended care setting, and also make the occasional home visit.  In all areas of the continuum, the Case Manager will focus on ensuring that the patient is receiving the right care in the right setting as efficiently as possible.   This position also includes a heavy emphasis on teaching and coaching, to maximize the patient’s ability to manage their conditions successfully and to play an active role in their care.  This is a full-time M-F position.  Qualifications:  Unrestricted Ma. RN licensure with 2 years of inpatient case management experience preferred.  Managed Care background and knowledge of Interqual criteria desirable.  Extended care and Home care experience a plus. We offer a comprehensive 6-week preceptor based orientation, as well as a competitive compensation package and free parking.

INPATIENT CASE MANAGER-Mount Auburn Hospital, Cambridge, MA

 

Mount Auburn Hospital is a 190 bed acute care teaching hospital affiliated with Harvard Medical School.  We currently have an opening for a Case Manager on one of our medical floors.   Responsibilities include assessing each patient to ensure appropriateness of care setting, and collaborating with the patient, family, health care team, and payer to create a discharge plan that meets the patient's needs.  This is a full-time M-F position with occasional evening beeper coverage.   

Qualifications:  Unrestricted Ma. RN licensure with 2 years of recent inpatient case management experience preferred.  Managed Care background and knowledge of Interqual desirable.  At least 3 years of recent med/surg experience in an acute setting required. This is an excellent opportunity for the clinical nurse who is looking to move into the field of Case Management.   We offer a 6-week preceptor based orientation, as well as a competitive compensation package and free parking.
