Manager to assist our physicians in managing the
care of our BCBS patients. In this newly created
position, the Case Manager will follow patients in
the hospital, extended care setting, and also make
the occasional home visit. In all areas of the
continuum, the Case Manager will focus on ensuring
that the patient is receiving the right care in the
right setting as efficiently as possible. This
position also includes a heavy emphasis on teaching
and coaching, to maximize the patient’s ability to
manage their conditions successfully and to play an
active role in their care. This is a full-time M-F
position. Qualifications:
Unrestricted Ma. RN licensure with 2 years of

Auburn Hospital is a 190 bed acute care teaching
hospital affiliated with Harvard Medical School. We
currently have an opening for a Case Manager on one
of our medical floors. Responsibilities include
assessing each patient to ensure appropriateness of
care setting, and collaborating with the patient,
family, health care team, and payer to create a
discharge plan that meets the patient's needs. This
is a full-time M-F position with occasional evening
beeper coverage.
Qualifications: Unrestricted Ma. RN licensure
with 2 years of recent inpatient case management
experience preferred. Managed Care background and
knowledge of Interqual desirable. At least 3 years
of recent med/surg experience in an acute setting
